What Is Histamine Intolerance?

Histamine is a compound naturally produced by your body and found in many foods. It plays crucial roles in immune function, digestion, and brain chemistry. Under normal circumstances, two key enzymes—diamine oxidase (DAO) and histamine N-methyltransferase (HNMT)—efficiently break down histamine, maintaining healthy balance (Maintz & Novak, 2007).

However, when these enzymes aren’t functioning optimally due to genetic variations, nutrient deficiencies, or other factors, histamine can accumulate in your body. This buildup triggers what experts call histamine intolerance, leading to a cascade of symptoms across multiple body systems (Comas-Basté et al., 2020).

Think of it this way: your body’s histamine “bucket” is overflowing because the drain (your enzymes) isn’t working well enough to keep up. The overflow spills into different systems, creating diverse symptoms that can feel overwhelming and disconnected.

One of the most validating aspects of understanding histamine intolerance is realizing that your symptoms aren’t “all in your head.” Research has documented extensive connections between histamine dysregulation and conditions affecting virtually every body system.

Digestive System

Gastrointestinal symptoms are often the first clue. You might experience bloating, abdominal pain, diarrhea, constipation, nausea, or that uncomfortable feeling of fullness after eating (Schnedl et al., 2019). These symptoms occur because histamine affects gut motility, stomach acid production, and intestinal permeability. If you’ve been diagnosed with irritable bowel syndrome (IBS) but haven’t found relief, histamine intolerance might be the missing piece of your puzzle.

Neurological and Mental Health

The brain is particularly sensitive to histamine fluctuations. Many people with histamine intolerance struggle with headaches and migraines (Izquierdo-Casas et al., 2019), brain fog, sleep disturbances, anxiety, depression, and mood swings (Qian et al., 2022). The histamine system in your brain regulates wakefulness, cognition, and emotional responses, so when it’s out of balance, these functions suffer.

Importantly, research has linked HNMT genetic variations to conditions including schizophrenia, ADHD, and Parkinson’s disease (Yang et al., 2015), highlighting just how crucial proper histamine metabolism is for brain health. Studies show that patients with severe atopic dermatitis have significantly reduced DAO enzyme activity (Ionmescu, G and Kiehl, R, 1989), and a low-histamine diet has helped improve symptoms in many cases.

Respiratory System

Chronic nasal congestion, runny nose, sneezing, asthma symptoms, and difficulty breathing may all stem from histamine’s effects on your respiratory system (Vassilopoulou, et al, 2020). If you feel like you have constant allergies but allergy testing is negative, histamine intolerance deserves investigation.

Reproductive Health

Women often notice histamine-related symptoms fluctuate with their menstrual cycle. Severe menstrual cramps, cycle-dependent headaches, PMS, and PMDD can all be connected to histamine (Maintz et al., 2011). This happens because estrogen and histamine have a bidirectional relationship: estrogen increases histamine production, and histamine increases estrogen production, creating a challenging cycle.

Movement and Sleep

Restless legs syndrome (RLS) and periodic limb movements have been linked to histamine dysregulation (Weinstock et al., 2021). The connection is so strong that medications affecting histamine can worsen RLS symptoms, while addressing histamine imbalance can provide relief.

Two primary enzyme systems control histamine levels in your body, and understanding them is key to your healing journey.

Diamine Oxidase (DAO)

DAO is your gut’s primary histamine-degrading enzyme. It breaks down histamine from food before it enters your bloodstream (Maintz & Novak, 2007). When DAO function is compromised, dietary histamine becomes problematic.

DAO requires several nutrient cofactors to function properly:

  • Vitamin B6 (pyridoxal phosphate)
  • Copper
  • Vitamin C
  • Zinc
  • Vitamin B2 (riboflavin/FAD)
  • Iron

Research shows that supplementing with vitamin C can decrease serum histamine concentrations in patients with allergic and non-allergic diseases (Hagel et al., 2013). This is encouraging news because it means simple nutritional interventions can make a real difference. DAO enzyme genetic variant can be tested (usually a saliva swab) and supplemented in animal and plant-based versions. When taken with meals, DAO can substantially reduce reactivity to foods.

Histamine N-Methyltransferase (HNMT)

The crucial piece many people miss about histamine intolerance: HNMT functions primarily in the central nervous system and intracellular spaces (Yoshikawa,m et al, 2019). Unlike DAO, this enzyme depends on a different set of cofactors:

  • S-adenosylmethionine (SAM-e)
  • Vitamin B12
  • Folate
  • Magnesium

Genetic variations in the HNMT gene can significantly impact enzyme activity (Preuss et al., 1998). Some polymorphisms reduce enzyme function, leading to histamine accumulation. Importantly, mutations in HNMT have been associated with intellectual disability (Heidari et al., 2015), underscoring the enzyme’s critical role in development and cognition. HNMT enzyme genetic variant can be tested (usually a saliva swab), but not supplemented directly.

Connected Conditions Worth Understanding

Histamine intolerance doesn’t exist in isolation. Several related conditions share common mechanisms, and understanding these connections can help you and your healthcare team develop a comprehensive treatment approach.

Mast Cell Activation Syndrome (MCAS)

MCAS involves inappropriate mast cell degranulation, releasing excessive histamine and other inflammatory mediators (Afrin et al., 2021). Many people with histamine intolerance have underlying MCAS, and addressing both conditions together (and the root causes) often yields better results than treating either alone.

Fibromyalgia

Research reveals increased mast cells and histamine in fibromyalgia patients (Tsilioni et al., 2016), with one study finding 74.5% of Spanish women with fibromyalgia had genetic DAO deficiency (Okutan, G, 2023). This connection explains why some fibromyalgia patients improve with mast cell stabilizers like ketotifen.

Chronic Fatigue Syndrome (ME/CFS)

Persistent infection and hypersensitivity to histamine’s immune-suppressive effects may contribute to CFS pathophysiology (Dechene et al., 1993). Stress-induced mast cell activation is proposed as a causative mechanism, and histamine’s effects on wakefulness-promoting neurons may contribute to the profound fatigue characteristic of this condition.

Autism Spectrum Disorder (ASD)

Emerging research shows altered expression of histaminergic genes in individuals with ASD (Wright et al., 2017), and mast cell dysregulation may be involved in 75% or more of autism cases (Theoharides et al., 2016). Histamine dysregulation may contribute to cognitive impairments, sensory issues, mood changes, and gastrointestinal symptoms commonly seen in ASD.

Type 2 Diabetes and Insulin Resistance

Histamine influences glucose metabolism and insulin release (Pini, A. et al., 2016). Studies in knockout mice show that blocking H1 and H2 receptors reduces post-exercise insulin sensitivity by 25% (Pellinger, et al, 2013), demonstrating histamine’s crucial role in metabolic health.

Inflammatory Bowel Disease (IBD)

Both Crohn’s disease and ulcerative colitis are associated with elevated histamine and reduced DAO activity (Raithel et al., 1999). Histamine plays a significant role in intestinal inflammation and immune regulation.

Vitamin B6 (Pyridoxal-5-Phosphate)

Vitamin B6 is an essential cofactor for DAO enzyme function. The active form, pyridoxal-5-phosphate (P5P), is particularly important. Good food sources include wild-caught fish, organic poultry, chickpeas, and bananas. Many people benefit from P5P supplementation, especially if they have genetic variants affecting B6 metabolism.

Vitamin C

This powerful antioxidant directly supports DAO function and has been shown to reduce histamine levels (Hagel et al., 2013). Fresh sources include red bell peppers, broccoli, and citrus fruits (though some people with histamine intolerance find citrus problematic). Supplementation with buffered vitamin C can be very helpful.

Copper

Copper is another crucial DAO cofactor. Good sources include grass-fed beef liver, shellfish, seeds, and nuts. However, copper supplementation should be approached carefully and ideally under professional guidance, as copper and zinc must be balanced. Too much copper without adequate zinc can cause problems.

Zinc

Zinc supports DAO function and helps modulate immune responses. Oysters, grass-fed beef, pumpkin seeds, and chickpeas are excellent sources. Many people are zinc-deficient, and supplementation often helps, but it should be balanced with copper intake.

B Vitamins for Methylation

Magnesium

Magnesium supports histamine metabolism and has natural calming effects. It’s involved in over 300 enzymatic reactions in the body. Most people are magnesium deficient even when bloodwork looks normal—up to 95% of your body’s magnesium is stored inside cells, where standard blood tests can’t measure it. Good sources include leafy greens, pumpkin seeds, dark chocolate, and avocados. Magnesium glycinate is often the best-tolerated supplement form, without causing gastrointestinal issues.

Iron

Iron deficiency can impair DAO function and has been specifically linked to restless legs syndrome in the context of histamine dysregulation. However, iron supplementation should only be done based on testing, as excess iron can be harmful. Good food sources include grass-fed red meat, organ meats, and dark leafy greens. Non-food sources include cooking in cast-iron pans.

Practical Steps Forward

If you suspect histamine intolerance, here are encouraging, practical steps you can take:

1. Get Proper Testing

Work with a qualified practitioner to assess your DAO enzyme activity, check for nutrient deficiencies, and consider genetic testing for DAO and HNMT variants. Some practitioners also measure serum histamine, although this can be tricky to interpret. Ellison Clark, MS RDN LDN

Ellison is a licensed Registered Dietitian with a strong background in endocrinology and hormones. Originally from North Carolina, Ellison is a proud Carolina Tarheel alum, and met Dr. Brown while working in community nutrition at the foodbank where she was a program coordinator for Cooking Matters at the Store grocery store tours and educational programs. Ellison completed her Masters in Nutrition from Meredith College in Raleigh.

Her early career includes clinical nutrition positions with Atrium Health before settling into private practice to focus on endocrinology and hormone health. Her continuing education training has focused on PCOS, nutrition and gut health.

Ellison sees clients in person in Wilmington, and via telehealth.

Dr. Jill Brown, DCN RDN LDN IFNCP CLT

Integrative Dietitian and Founder

Dr. Jill Brown is a licensed Registered Dietitian and Board Certified Functional Nutrition Certified Practitioner. Originally from the Midwest, Dr. Brown completed undergraduate studies in Education at Texas Woman’s University, a Masters in Nutrition from Meredith College, and a Doctorate in Clinical Nutrition from Maryland University of Integrative Health.

Her early career started in Neonatal Intensive Care at both UNC and Duke Hospitals and Diabetes Self Management Program. She later took on a leadership role with one of the Feeding America food banks as Director of Nutrition, setting into clinical roles at Duke Integrative Medicine and Kaizen Nutrition & Wellness.

She has extensive training in integrative and functional nutrition as well as several complementary modalities including Clinical Aromatherapy, Polyvagal Theory, Herbal Medicine, Functional Women’s Health, Heart Math®, and Reiki Energy Healing.

Dr. Brown sees clients at her home office in Mebane, and via telehealth.
